Formstack pricing explained, plan by plan
Formstack publishes two prices and two quotes. Forms and Suite have a price on the pricing page; Salesforce Solutions and Enterprise Solutions say Custom Pricing and send you to the sales team. Every number below was read from Formstack's own pricing page on 2026-10-10, in US dollars, with the annual and monthly toggle both checked.
This is not a tool for someone who wants a free contact form. The cheapest plan is $83 a month on annual billing, or $99 if you pay month by month, and it comes with one builder user. If you only need a form that collects answers from a few hundred people, most of what you pay for here is capacity and features you may never open.

What Formstack does well
Built for approvals and routing
Formstack's Forms page lists group approvals, step logic and conditional routing. If a submitted form has to travel through several people before anything happens, that is a workflow product, and a simple form builder does not replace it.
Prefill, AI form creation and 300+ integrations
The Forms page lists SSO-powered form prefill and an AI form builder, and the pricing table says Forms and Suite connect to 300+ integrations. For a team that pushes data into a CRM or a document store, those links are the main thing the plan pays for.
Documents and eSignature in the same plan
Suite adds document generation and unlimited eSignatures to the form builder. If a form ends in a contract or a signed PDF, one vendor for all three steps is a real convenience.
Compliance and Salesforce paths
The page offers a HIPAA and compliance add-on, Salesforce-native forms, SSO and custom SLAs on the enterprise side. A small tool cannot stand in for these when a security review is waiting.
Capacity is what the price buys
Formstack's plans differ mostly in how much you can run. Forms gives you 1 builder user, 25 forms and 1,000 submissions per form. Suite gives 3 builder users, 100 forms and 5,000 submissions per form, and adds two other products.
The submission limit is per form, not per account. Twenty-five forms at 1,000 each is a generous ceiling for most small teams. A single popular form is another matter: a registration form that crosses 1,000 submissions leaves you with the add-on route the FAQ mentions, or the jump to Suite.
Annual or monthly billing
The pricing page shows a toggle and the line Save 2 months with annual plans. Forms is $83 a month paid annually or $99 paid monthly, which is $996 against $1,188 over twelve months. Suite is $250 a month paid annually or $299 monthly, which is $3,000 against $3,588.
Payment is by Mastercard, Visa, Discover or American Express, and the FAQ says annual invoices are available for certain plans. It also says the nonprofit discount applies to invoices but the annual discount does not.
What it adds up to over one, two and three years
Forms on annual billing is $996 after a year, $1,992 after two and $2,988 after three. On monthly billing it is $1,188, $2,376 and $3,564. Suite on annual billing is $3,000, $6,000 and $9,000, and on monthly billing $3,588, $7,176 and $10,764.
These totals assume today's prices and one account with no add-ons. The page does not print the price of extra users, extra submissions or the HIPAA add-on, so a real bill can be higher. Formstack can also change its prices at any time.
The free trial
The FAQ describes a 14-day free trial of the core Forms product, with no credit card required, and full access to build forms during that time. At the end you upgrade to a paid plan to continue. There is no permanent free plan on the page.
Who should stay on Formstack, and who can look elsewhere
Stay if your forms need HIPAA handling, approvals across several people, Salesforce, documents or eSignatures, or a vendor with SSO and an SLA. Those are the reasons the price exists, and a $25 form builder does not cover them.
Look elsewhere if you mostly need a branded form with skip logic, an embed, a response table and a CSV export. MangoForm costs $25 once, has no cap on forms or responses, and has a read-only API and a webhook on every response. It does not take payments or file uploads, has no compliance certifications and no approval workflow, so a regulated or multi-step process is not its job.
